What kinds of questions would you expect to see in a comment card for a delivery service?

I'm starting a delivery service for furniture and other household items. I'm trying to make a comment card to get feedbac from my clients.

Public Comments

1. Was your item received in a timely manner? Was your item received in perfect condition? Is there any way we may improve our service? Would you recommend us to your friends?
2. Was the delivery made at the time expected/promised? Were the goods delivered in good condition? Was the delivery person polite and considerate?
3. Was delivery performed in a timely manner? Was driver/ personnel uniformed and polite? Was equipment clean and appear to be in good running order? Would you use our service again? Would you recommend our service to friends/ colleagues?
4. Were the available delivery hours convenient? Was the delivery on time? Did the product arrive in perfect condition? Was the delivery person professional and polite? How would you rate the delivery cost? How would you compare this delivery to competitor businesses? How was your overall service? Would you use this service again? Would you recommend this service to friends and family? I would also print the card on a pre-postage paid card and offer a discount or small gift for completing the card.
