What is the correct format of US postal addresses?

I installed a new program to help me with mailings to USA. Now I am confused, because the program gives me a different address format than the one I am used to. New address format: Mr <first name> <last name> 1234, <street> <city> 12345-6789 USA Old address format: Mr <first name> <last name> 1234, <street> <city> <state> 12345 USA Which one is correct?

1. The second one. You have to include a State in any address.

Portals and directories are like phone books, and they often provide a lot of traffic because they tend to rank high on Google.
