How would you start a wheelchair accessible van/delivery service?

I live in an area that is rurally spread out..The 2 major towns are 14 miles apart with a lot of little places in between and all around. The 2 towns have one taxi service each and they're both privately owned by 1 person. There is a van service but because my area is so big they're on a limited schedule with limited funding they stop at 6pm and don't run at all on weekends. They offer limited door to door service and even if you give advance notice chances are they won't be available unless you give at least 2 weeks notice..The elderly the indigent and the disabled have no way to make medical appointments go to jobs or get groceries..How would one go about starting at the very least, a service where passingers can be picked up door to door where groceries, perscriptions can be picked up from the stores and delivered to the customers who can't get out? A business plan, funding, the works.
