What is the specific form I use for canceling temporary mail forwarding with US postal service?

I signed up online on Thursday for temporary mail forwarding with the United States postal service. Now I am trying to CANCEL my request, and can't seem to find any way to do it. I tried to find an online link; NO LUCK. I phoned the 800 number for the post office and they said to fill out the same change-of-address card as you would do for any move, and then fill in the blank for canceling--but there IS no blank for canceling. I am trying to find online a pdf form if I have to do this in person, because I will be in a real rush on Monday, trying to accomplish other things with my move. thank you for any links/info that would help. PS Please also let me know if you know of a better category for this question; I had trouble finding the place for it!

1. I don't think there is any online form. When this happened to me, I had to go to the post office in person, and they pulled down this flimsy piece of paper with very little printing on it and had me quickly fill it out. It didn't have much content to it at all. I wish I had taken the time to note if it actually had a form name/number to it, but sorry I didn't. It moved very quickly, though, if you can make it there in person.
